Open Enrollment Applications Available at New Lebanon Local School District

Parents of students in kindergarten, 3rd, and 5th – 12th grades may apply for open enrollment for the 2012-2013 school year. Applications will be accepted April 16, 2012 – May 31, 2012. Applications must be submitted in person at the Board Office, which is located in Dixie High School. Applications are available online here or at the Board Office.

Approval of applications for open enrollment in kindergarten, 3rd, 5th – 12th grades are at the discretion of the Superintendent in accordance with the district’s policies and administrative regulations. Acceptance is based on class size and program offerings. For additional information, please call the Board Office at (937) 687-1301 ext. 1.





Dixie Elementary School Kindergarten Registration is planned for the 2012-2013 school year. The month of April has been designated as Kindergarten registration month. Children must be five years of age by August 1st to enter kindergarten. The registration process will take approximately one hour. It is not necessary for the kindergartener to be present. Parents will receive all forms at registration and they will need to be completed at that time. You will need to bring names and telephone numbers of doctors, dentist and emergency contacts with you so that you can fill out all information.

You will be asked to present at this time a list of the following:

1. An official Birth Certificate. A hospital “record of birth” is not acceptable; 2. Child’s social security number; 3. Current immunization records; 4. Any custody or guardianship papers applicable; 5. Proof of residency (mailing portion of a utility bill). After all of the official paperwork is completed and turned in, you will be assigned a screening time (for a later date) that the student must attend. If you have any questions, please contact the Elementary Office at 687-3511 ext. 200.

*The New Lebanon Local Schools shall not admit to the first grade any child who has not successfully completed kindergarten unless approved by the district’s pupil personnel services committee. O.R.C. 3321.01(D).
